The Airedale & Craven Community Specialist Palliative Care Team sees patients who have active, progressive and usually advanced disease for which the prognosis can be limited (although it can be several months/years) and the focus of care is on quality of life.
Our team consists of:
Community Team Leader & Clinical Nurse Specialist: Lizzie Procter
Clinical Nurse Specialists: Joanna Longden, Sue Gee, Judith Nunn, Valerie Bone, Tina Hayton
Consultant in Palliative Medicine: Dr. Mike Hughes
Staff grade Doctor: Dr. Pam Selby.
Extended team members: physiotherapist, social workers, counsellor, complementary therapist & secretary
The service offers:
Specialist advice on pain and symptom management.
Emotional, psychological and social support for the patient and their family/carers.
Close liaison with primary health care team members.
Close liaison with the hospital palliative care team.
Education and staff support.
Appropriate referral to relevant support groups and other services.
Who can be referred:
Any patient with a potentially life limiting disease - eg cancer, MND, MS and end-stage cardio-respiratory disease.
When to refer:
Early referral is encouraged, enabling support mechanisms for the patient and family to be established. However, patients can be seen at any stage, from diagnosis to very advanced disease.
How we work:
All new referrals are discussed, contacted and, if appropriate, assessed within their own home environment. They are then seen again as necessary.
Any advice offered by the team is discussed with the GP and district nursing teams.
All new referrals are discussed daily and allocated.
We provide education in palliative care in both a formal and informal setting.

Referrals are accepted for patients and carers from Airedale PCT and a section of Bradford South and West PCT as well as from the Craven area.
