The Hospice at Home Team work alongside existing community services and the out-of-hours service to provide nursing and welfare benefits advice.
Referral criteria:

Patients over 16 years of age.

Registered with a GP or resident in the Airedale PCT area.

Suffering from a life-limiting illness and reasonably expected to die within six months.
Aims:

To give realistic choices surrounding the place of care and death.

To enable the patient to achieve a dignified symptom-controlled peaceful death in the place of their choice, whilst supporting family and carers and maximising the individual's quality of life.

- Collaborative working.
- Access to a range of specialist professionals and equipment.
