The Palliative Care Team at Airedale Hospital sees patients with active, progressive, far-advanced disease, for whom the focus of care is the quality of life, and in whom the prognosis is limited (although sometimes this may be several months/years).  Such care includes consideration of the families’ needs before and after the patient’s death.

The team consists of a part time consultant and two palliative care nurse specialists with links to the psychology and chaplaincy services.

The service offers:

Advice on pain and symptom control.
Support with complex discharge/transfer arrangements.
Education in palliative care.
Close liaison with hospices.
Emotional support for patient and family.
Support for staff.
Information on relevant support groups and other services.

Who can be referred:

Any patient with life threatening disease – eg cancer, MND, end-stage cardio-respiratory disease.

When to refer:

Early referral is encouraged, enabling support mechanisms for the patient and family to be established.  However, patients can be seen at any stage, from diagnosis to very advanced disease.

How we work:

New referrals are assessed by the Macmillan Nurse Specialist or Consultant and seen again at appropriate intervals.  Other team members see patients as necessary.
Any advice offered by the team is discussed with the ward medical and nursing staff.  The team meets weekly to discuss all patients referred.
We welcome requests to provide education in palliative care and formal/informal teaching can be arranged for medical, nursing or paramedical staff.
