Further Details about Mental Health Services

 

Primary Care Mental Health Teams

 

Services Provided:

  • Support and advice to professionals working with people with mild-moderate mental health problems.  These may be caused by low mood,ill  health, bereavement, traumatic events or relationship problems.
  • Assessment of patients referred as to whether and what intervention is needed.
  • Where appropriate, a number of therapeutic sessions over a short period of time.

 

Referral Criterion:

Anyone over 16 (not in full time education) registered with a Bradford and Airedale Primary Care Trust GP and with mild to moderate mental health difficulties.

 

Making a Referral:

Referrals are accepted from primary health care teams and voluntary organisations.  It is advisable to discuss referral
with the GP first.

 

Adult Community Mental Health Teams (for those of working age)

         

Services Provided:

  • Assessment of the severity of mental health problems, risk, complexity of needs and the ability to function.  Mental health difficulties may include – a severe level of depression, ideas of suicide and self-harm, psychotic illness e.g. paranoia, schizophrenia, bipolar disorder, manic depression.
  • Specialist mental health interventions, for those who may need back up support from a Community mental Health Nurse.
  • Advice re; medication.

 

Referral Criterion:

People aged 16-65 who have or may be developing a severe and enduring mental health problem.

 

Making a Referral:

Please call if you wish to discuss a potential referral.  There is a named CPN or Social Worker available each day for telephone advice and consultation.
The duty worker will triage a referral to the most appropriate team.  This may be the Crisis Resolution Team, Rapid Access Clinic or Community Mental Health Team.  Formal referrals should be made by letter or fax.  It is important that there is good preparation before referral is made and the person is clear on what will happen next. 

Available Monday to Thursday 9am - 5pm and Friday 9am - 4.30pm.

 

Crisis Resolution Teams for Working Aged Adults

 

Services Provided:

  • Teams offer a home treatment service as an alternative to hospital for people who are experiencing a mental health crisis and are of working age.
  • Provide short-term support in peoples’ own homes, 24 hours a day, 7 days a week.

Two Teams:

Airedale & Bradford North

Bradford City, South & West

 

Making a referral:

Referrals are only accepted by either a mental health professional or GP.

 

A & E Mental Health Liaison Service

 

Bradford Service:

  •   Mental health assessment service to adults attending A & E who need a mental health assessment.
  •   Mental health assessment for patients on a medical ward following a self harm episode.

For all other mental health assessments contact the community mental health team.

Airedale Service:

  •   Will provide an assessment for any adult on the wards who has mental health needs.

 

Hospital Services (Inpatients)

 

  •  Provide services for people with serious, acute mental health problems.
  •  For working aged adults, referral to the units is via the Crisis Resolution Teams.  Other services have different referral criteria – speak to the service direct.

 

Older Adult Community Mental Health Services

 

Services Provided:

  •  Assessment of the severity of mental health problems, risk, complexity of needs and ability to function that develop in old age or that are associated with old age.  Mental health difficulties may include - a severe level of depression, ideas of suicide and self-harm, psychotic illness e.g. paranoia, schizophrenia, bipolar disorder, manic depression, extreme issues associated with dementia.
  • Specialist mental health interventions, for those who may need back up support from a Community Mental Health Nurse, Occupational Therapist, Social Worker, Physiotherapist or Doctor.
  • Advice re: medication, meeting complex needs, identifying packages of care.

 

Referral Criterion:

People aged over 65, or people of any age who have dementia, and have or may be developing a severe and enduring mental health problem.

 

Making a referral:

 Please call the relevant CMHT if you wish to discuss a potential referral.

 Available Monday to Friday 9am – 5pm.

 

Social Services Emergency Duty Team

 

Offer emergency out of hours contact point.  Monday to Friday 5pm – 9am, Saturday & Sunday 24 hours per day.

 

Children & Young People

 Airedale CAMHS

Services Provided:

For children and young people, their families or carers who are experiencing emotional, behavioural or mental health problems.

Referral Criterion:

For those in full time education and up to age 18.

Making a Referral:

Need referral by GP, hospital doctor or school.

 

Bradford CAMHS

 

Services Provided:

Provide specialist mental health service.

Referral Criterion:

For those up to age 16 or old if in full time education.

Making a Referral:

Need referral by doctor, health visitor, social worker or educational social worker.
